ABSTRACT

Introducción: La hipertensión arterial (HTA) representa el principal factor de riesgo individual, con mayor carga a nivel mundial de enfermedades cardiovasculares (ECV). En nuestro país, algunos trabajos epidemiológicos han mostrado marcadas diferencias en las prevalencias de estos factores de riesgo de acuerdo con la población evaluada. Sin embargo, no hay estudios epidemiológicos de evaluación de factores de riesgo cardiovascular exclusivos referentes a barrios vulnerables con muy bajos recursos económicos, socioculturales y poca accesibilidad a los sistemas de salud. Materiales y métodos: Estudio observacional de corte transversal multicéntrico en habitantes de comunas vulnerables de muy bajos recursos, como asentamientos populares y barrios carenciados con muestreo aleatorizado simple de casas. Se realizaron tomas de presión arterial (PA), medidas antropométricas, así como cuestionarios epidemiológicos, económicos y socioculturales. Se describen los hallazgos: prevalencia, conocimiento y control de la PA en las distintas regiones. Se efectuó una regresión logística para determinar las variables independientes a los resultados principales. Resultados: Se analizaron 989 participantes. La prevalencia de HTA global fue de 48,2%. Un total de 82% tenía un índice de masa corporal (IMC) >25 kg/m2. De estos pacientes, 45,3% tenían menos de seis años de educación. Este último aspecto se asoció a mayor prevalencia de HTA de forma independiente. De los hipertensos, 44% desconocían su padecimiento y solo en 17,2% estaba controlado, asociándose esto a tener obra social (OS) y mayor nivel educativo. Únicamente 24% estaban bajo tratamiento combinado. Conclusión: La prevalencia de HTA en barrios vulnerables es elevada, superando a la de otros estratos sociales con niveles de conocimiento, tratamiento y control de la HTA bajos, similar a otras poblaciones. Se detectó un uso insuficiente de la terapia combinada.

ABSTRACT

We hypothesized that a poorer cardiovascular health status is related to a higher risk of hypertension-mediated organ-damage (HMOD) or hypertension-related comorbidities (HRC). We assessed the relationship between cardiovascular health metrics (CVHM) and HMOD-HRC in 243 hypertensive patients from primary care center followed for two years. We recorded the baseline CVHM score (Life's Simple 7) plus clinical data, including prevalent and incident HMOD-HRC, hospitalization and mortality. The prevalence of ideal CVHM scores was very low in both men and women. The patients with healthier CVHM scores were younger, and had a lower prevalence of diabetes, cardiovascular disease and chronic kidney disease. We recorded 264 cases of HMOD-HRC (225 at baseline and 39 during follow-up). Nine patients died and 64 had any-cause hospitalization during follow-up. A lower prevalence of HMOD-HRC and unfavorable outcomes was observed as the number of ideal CVHM increased (P<0.05). Multivariate logistic regression adjusted for confounders showed a lower CVHM score (0–1) was associated with increased odds of HMOD-HRC (4.04, 95% CI 1.26–12.94; P=0.019) and composite endpoint (HMOD-HRC, death or all-cause hospitalization) (3.43, 95% CI 1.19–9.92; P=0.023). Individual components were less predictive than the cumulative CVHM score. Few hypertensive patients in this urban population had ideal CVHM scores. An inverse relationship between scores and outcomes (HMOD-HRC, death or hospitalizations) was observed. Interventions to increase this score may improve prognosis among community-based hypertensive patients (AU)


Nuestra hipótesis fue que un estado de peor salud cardiovascular está relacionado con un mayor riesgo de daño orgánico provocado por la hipertensión (DOP HTA) o de comorbilidades relacionadas con la hipertensión (CRHTA). Evaluamos la relación entre las métricas de la salud cardiovascular (MSCV) y el DOP HTA-CRHTA en 243 pacientes hipertensos procedentes de un centro de atención primaria, a quienes se realizó un seguimiento durante 2 años. Registramos la puntuación basal de MSCV (Life's Simple 7) y los datos clínicos, incluyendo DOP HTA-CRHTA prevalente e incidental, hospitalización y mortalidad. La prevalencia de puntuaciones MSCV ideales fue muy baja tanto en hombres como en mujeres. Los pacientes con puntuaciones MSCV más saludables fueron más jóvenes y tuvieron una menor prevalencia de diabetes, cardiopatías y enfermedad renal crónica. Registramos 264 casos de DOP HTA-CRHTA (225 al inicio y 39 durante el seguimiento). Nueve pacientes fallecieron y 64 fueron hospitalizados por cualquier causa durante el seguimiento. Se observó una menor prevalencia de DOP HTA-CRHTA y resultados no favorables a medida que aumentaba el MSCV ideal (p<0,05). La regresión logística multivariante ajustada a los factores de confusión reflejó una menor puntuación MSCV (0-1) asociada a un incremento de la odds ratio de DOP HTA-CRHTA (4,04, IC 95% 1,26-12,94; p=0,019) y un resultado compuesto (DOP HTA-CRHTA, muerte u hospitalización por cualquier causa) (3,43, IC 95% 1,19-9,92; p=0,023). Los componentes individuales fueron menos predictivos que la puntuación MSCV acumulada. Pocos pacientes hipertensos de esta población urbana tuvieron puntuaciones MSCV ideales. Se observó una relación inversa entre las puntuaciones y los resultados (DOP HTA-CRHTA, muerte u hospitalizaciones). Las intervenciones para incrementar esta puntuación pueden mejorar el pronóstico entre los pacientes hipertensos con base comunitaria (AU)

ABSTRACT

Objetivo:Evaluar si la aparición de una situación de crisis como la que ha supuesto la pandemia por el SARS-CoV-2, junto a los cambios organizativos adoptados en Atención Primaria, han influido en la implementación de actividades preventivas cardiovasculares en pacientes de 40 a 74 años. Diseño: Estudio descriptivo multicéntrico retrospectivo durante tres años (2019-2022) en Atención Primaria.Ámbito: 35 centros de salud de Atención Primaria de la Dirección Asistencial Norte de Madrid. Participantes: Se incluyeron 1.008 pacientes de ambos sexos entre 40 y 74 años con diagnósticos de hipertensión, diabetes mellitus y/o dislipemia. Método: Se recogen a partir de la historia clínica informatizada las siguientes variables: actividades preventivas sobre el estilo de vida (consumo de tabaco, alcohol, consumo de dieta mediterránea y práctica de ejercicio) considerando óptimo tres de los cuatro parámetros; datos de exploración (registro de tensión arterial) y registro de analítica (glucemia, HbA1c, colesterol total, HDL, LDL) considerando óptimo cuatro de los cinco parámetros. Se analizan diferencias entre tiempo de prepandemia (15/03/2019-14/03/2020), pandemia (15/03/2020-14/03/2021) y transición (15/03/2021-14/03/2022). Análisis estadístico: Prueba de McNemar para comparar las variables principales entre los periodos de estudio. Resultados: Se recogen datos de 1.008 pacientes. El registro de actividades preventivas sobre el estilo de vida fue de 180 pacientes (17,9%) (IC 95%: 0,155-0,204) en prepandemia, 29 pacientes (2,9%) (IC 95%: 0,019-0,041) en pandemia y 55 pacientes (5,5%) (IC 95%: 0,041-0,070) en la etapa de transición (p < 0,05). La exploración estaba registrada en 393 pacientes (39%) (IC 95%: 0,360-0,421) en la prepandemia, 133 pacientes (13,2%) (IC 95%: 0,112-0,154) en pandemia y en 218 pacientes (21,6%) (IC 95%: 0,191-0,243) en la transición (p < 0,05).(AU)

ABSTRACT

OBJECTIVE: The objective of the study is to describe the clinical and epidemiological characteristics of our patients with elevated Lp(a). MATERIALS AND METHODS: A descriptive cross-sectional study was conducted on 316 patients with elevated Lp(a) (>125 nmol/L) in a random sample between January and August 2022. We measured epidemiological, anthropometric, clinical and laboratory variables (lipid metabolism parameters, carbohydrates and hormones). RESULTS: Mean age of our sample subject's was 59 ± 15 years with 56% males. The average BMI was 27.6 kg/m2 (71% with elevated BMI). Elevated waist circumference was observed in 54.1% of men and 77.8% of women. 48% had hypertension, 30.7% had diabetes mellitus and 91.5% dyslipidemia. Only 39.7% of the patients had never smoked. The mean values of total cholesterol were 158 ± 45 mg/dl, LDL was 81 ± 39 mg/dl, HDL was 53 ± 17 mg/dl, Triglycerides were 127 ± 61 mg/dl, and Lp(a) was 260 ± 129 nmol/L. Regarding lipid lowering treatment, 89% were on statins, 68.6% on ezetimibe, and 13.7% on PCSK9 inhibitors. 177 patients (57,7%) had established cardiovascular disease (CVD), 16.3% had polyvascular disease, 11.7% had subclinical CVD, and 30.6% had no known CVD. Among patients with established CVD, 174 (98.3%) were on lipid-lowering treatment (97.2% on statins) and 86.4% were on antiplatelet therapy. The mean age of cardiovascular events was 55 ± 12 years in males and 60 ± 11 years in females. 65,1% of female and 56,2% of male patients suffered an early cardiovascular event. CONCLUSIONS: Patients with elevated Lp(a) are at very high cardiovascular risk, particularly for early cardiovascular disease.

Introducción: las alteraciones del control glicémico y los lípidos son causas principales de enfermedad cardiovascular. Sin embargo, los patrones metabólicos en el infarto agudo de miocardio no están bien establecidos. Objetivo: el propósito de esta investigación es describir las características metabólicas en pacientes admitidos con infarto de miocardio. Clasificamos los pacientes según el cociente HDL-c/LDL-c y perfiles lipídicos. Método: estudio observacional, descriptivo, retrospectivo en pacientes admitidos al Servicio de Cardiología con infarto agudo de miocardio durante un año. Resultados: 97 pacientes fueron incluidos, predominó el sexo masculino (67%) y el promedio de edad fue 59 años (SD, ±9.9). Las características metabólicas fueron las siguientes: mediana de hemoglobina glicosilada 6.2% (IQR, 5.8-7.5), glucemia en ayunas 107 mg/dL (IQR, 99-152), colesterol total 201 mg/dL (IQR, 165-223), colesterol asociado a lipoproteínas de baja densidad (LDL-c) 123 mg/dL (IQR, 96-146), colesterol asociado a lipoproteínas de alta densidad (HDL-c) 38.2 mg/dL (IQR, 31.8-45.4) y triglicéridos (TG) 161 mg/dL (IQR, 103-234). El cociente HDL-c/LDL-c <0.4 estuvo presente en 75% de los pacientes. Los patientes fueron clasificados según perfiles lipídicos, donde el perfil L-1 [HDL-c ≤40, LDL-c <150, TG <200] y L-2 [HDL-c >40, LDL-c <150, TG <200] en conjunto representaron la mitad de la muestra. Conclusiones: prediabetes, bajos niveles de HDL-c y un cociente HDL-c/LDL-c <0.4 fueron hallazgos frecuentes en infarto de miocardio. Objective: The purpose of this research is to describe the metabolic characteristics of patients admitted with myocardial infarction. Method: observational, descriptive, retrospective study in patients admitted to the Cardiology Department with acute myocardial infarction for one year. Results: 97 patients were included, males predominated (67%) and the mean age was 59 years (SD, ±9.9). Metabolic characteristics were as follows: median glycosylated hemoglobin 6.2% (IQR, 5.8-7.5), fasting blood glucose 107 mg/dL (IQR, 99-152), total cholesterol 201 mg/dL (IQR, 165-223), low-density lipoprotein cholesterol (LDL-c) 123 mg/dL (IQR, 96-146), high-density lipoprotein-associated cholesterol (HDL-c) 38.2 mg/dL (IQR, 31.8-45.4) and triglycerides (TG) 161 mg/dL (IQR, 103-234). The HDL-c/LDL-c ratio <0.4 was present in 75% of patients. Patients were classified according to lipid profiles, where the L-1 HDL-c ≤ 40, LDL-c <150, TG <200] and L-2 ]HDL-c >40, LDL-c <150, TG <200] together represented half of the sample. ABSTRACT

Las enfermedades cardiovasculares (Cv), son las causantes de la mayor parte de fallecimientos, como consecuencia de dislipidemia y enfermedad renal crónica (ERC). En Ecuador uno de cada cinco individuos padece de hipertensión arterial, patología que está directamente relacionada con las enfermedades cardiovasculares y la enfermedad renal crónica. Objetivo. Caracterizar el perfil de riesgo cardiovascular en pacientes con enfermedad renal crónica y su asociación con la presencia de dislipidemia, atendidos en el Hospital Homero Castanier de la ciudad de Azogues, durante el periodo de enero a diciembre de 2021. Materiales y Métodos. Se llevó a cabo una investigación de diseño observacional, descriptivo y retrospectivo donde se analizaron 104 historias clínicas de pacientes, correspondientes al periodo comprendido entre enero y diciembre de 2021. Para el cálculo del riesgo cardiovascular se empleó la tabla de predicción del riesgo AMR-D de la Organización Mundial de la Salud/Sociedad Internacional de Hipertensión para el continente americano categoría D. Los datos fueron analizados en el programa SPSS, mediante estadística descriptiva e inferencial. Resultados. De los datos analizados de los 104 pacientes con enfermedad renal crónica el 44,2% presentaron dislipidemia, al mismo tiempo el 74% de los pacientes en estudio manifestaron un riesgo cardiovascular bajo, seguido del 13,5% con riesgo moderado; destacando entre las principales comorbilidades la hipertensión arterial y la diabetes mellitus. Conclusiones. Se caracterizó el riesgo cardiovascular de los pacientes con ERC atendidos en el Hospital Homero Castanier de la ciudad de Azogues en el periodo enero - diciembre 2021, encontrando una baja prevalencia de desarrollar riesgo cardiovascular.


Cardiovascular diseases (CVD) are responsible for most deaths as a consequence of dyslipidemia and chronic kidney disease (CKD). In Ecuador, one out of every five individuals suffers from arterial hypertension, a pathology that is directly related to cardiovascular diseases and chronic kidney disease. Objective. To characterize the cardiovascular risk profile in patients with chronic kidney disease and its association with the presence of dyslipidemia, attended at the Homero Castanier Hospital in the city of Azogues, during the period from January to December 2021. Materials and Methods. An observational, descriptive and retrospective research design was carried out in which 104 clinical histories of patients were analyzed, corresponding to the period between January and December 2021. The AMR-D risk prediction table of the World Health Organization/International Society of Hypertension for the Americas category D was used to calculate cardiovascular risk. The data were analyzed in the SPSS program, using descriptive and inferential statistics. Results. Of the data analyzed, 44.2% of the 104 patients with chronic kidney disease presented dyslipidemia; at the same time, 74% of the patients in the study showed low cardiovascular risk, followed by 13.5% with moderate risk, with arterial hypertension and diabetes mellitus standing out among the main comorbidities. Conclusions. The cardiovascular risk of patients with CKD treated at the Homero Castanier Hospital in the city of Azogues during the period January-December 2021 was characterized, finding a low prevalence of developing cardiovascular risk.


As doenças cardiovasculares (DCV) são responsáveis pela maioria das mortes como consequência da dislipidemia e da doença renal crônica (DRC). No Equador, um em cada cinco indivíduos sofre de hipertensão arterial, uma patologia que está diretamente relacionada às doenças cardiovasculares e à doença renal crônica. Objetivo. Caracterizar o perfil de risco cardiovascular em pacientes com doença renal crônica e sua associação com a presença de dislipidemia, atendidos no Hospital Homero Castanier, na cidade de Azogues, durante o período de janeiro a dezembro de 2021. Materiais e métodos. Foi realizado um desenho de pesquisa observacional, descritivo e retrospectivo, no qual foram analisados 104 históricos clínicos de pacientes, correspondentes ao período entre janeiro e dezembro de 2021. A tabela de previsão de risco AMR-D da Organização Mundial da Saúde/Sociedade Internacional de Hipertensão para as Américas categoria D foi usada para calcular o risco cardiovascular. Os dados foram analisados no SPSS usando estatísticas descritivas e inferenciais. Resultados. Dos 104 pacientes com doença renal crônica, 44,2% apresentavam dislipidemia, enquanto 74% dos pacientes em estudo tinham baixo risco cardiovascular, seguidos por 13,5% com risco moderado; as principais comorbidades foram hipertensão e diabetes mellitus. Conclusões. Foi caracterizado o risco cardiovascular dos pacientes com DRC atendidos no Hospital Homero Castanier da cidade de Azogues no período de janeiro a dezembro de 2021, encontrando uma baixa prevalência de desenvolvimento de risco cardiovascular.

ABSTRACT

INTRODUCTION AND OBJECTIVES: The American Heart Association has recently developed the Life's Essential 8 (LE8) score to encourage prevention of cardiovascular disease (CVD). This study assessed the distribution of LE8 in the Spanish adult population and its association with all-cause and CVD death. METHODS: We used data from 11 616 individuals aged 18 years and older (50.5% women) from the ENRICA study, recruited between 2008 and 2010 and followed up until 2020 to 2022. The LE8 score includes 8 metrics (diet, physical activity, nicotine exposure, sleep health, body mass index, blood lipids and glucose, and blood pressure) and ranges from 0 to 100. The association of LE8 score with mortality was summarized with hazard ratios (HR), obtained from Cox regression. RESULTS: In total, 13.2% of participants (range, 6.1%-16.9% across regions) had low cardiovascular health (LE8 ≤ 49). During a median follow-up of 12.9 years, 908 total deaths occurred, and, during a median follow-up of 11.8 years, 207 CVD deaths were ascertained. After adjustment for the main potential confounders and compared with being in the least healthy (lowest) quartile of LE8, the HR (95%CI) of all-cause mortality for the second, third and fourth quartiles were 0.68 (0.56-0.83), 0.63 (0.51-0.78), and 0.53 (0.39-0.72), respectively. The corresponding figures for CVD mortality, after accounting for competing mortality risks, were 0.62 (0.39-0.97), 0.55 (0.32-0.93), and 0.38 (0.16-0.89). CONCLUSIONS: A substantial proportion of the Spanish population showed low cardiovascular health. A higher LE8 score, starting from the second quartile, was associated with lower all-cause and CVD mortality.

ABSTRACT

El objetivo es evaluar un seguimiento telemático (web o aplicación [app]) para pacientes con sobrepeso/obesidad y otros dos factores de riesgo cardiovascular: hipertensión arterial (HTA), dislipemia, diabetes mellitus (DM), sedentarismo, consumo tabáquico. Diseño: es un estudio de intervención con asignación aleatoria al grupo intervención (web o app) y al grupo control. Emplazamiento: centros de salud rurales y urbanos, docentes y no docentes, del Sector Zaragoza I. Participantes: 261 personas con índice de masa corporal (IMC) > 25 kg/m2 y otros dos factores de riesgo. Intervenciones: seguimiento de 18 meses con puntos de corte al inicio, 1, 6, 12 y 18 meses. Las mediciones principales son peso, tensión arterial, consumo tabáquico, eventos cardiovasculares, calidad de vida y bioquímica. Resultados: el grupo control finalizó el estudio en mayor proporción (79% versus 14%). El grupo control consiguió una pérdida del 8% de la mediana de peso al año y el grupo intervención un 5%. A los 6 meses, el grupo control logró disminuir el 7% la tensión arterial sistólica y el 5% la diastólica. La hemoglobina glicosilada (HbA1c) descendió un 1% en el grupo control y un 0,5% en el grupo intervención (test de Wilcoxon: 10; p = 0,089). El colesterol de lipoproteínas de baja densidad (cLDL) del grupo control descendió 9 mg/dL, y el del grupo intervención, 7 mg/dl (test de Wilcoxon: 1.089; p = 0,018). El hábito tabáquico disminuyó en todos los grupos (test de Wilcoxon: 21; p = 0,036). El grupo control presentó mayor prevalencia de eventos cardiovasculares. La calidad de vida mejoró en todos los grupos (test Wilcoxon: 979; p = 0,041). Conclusiones: las/los pacientes que acuden al centro de salud para un seguimiento de peso consiguen mejores resultados que si el seguimiento se hace de forma telemática.(AU)


The aim was to evaluate remote assistance (Web or App) for overweight/obese patients with two extra cardiovascular risk factors: hypertension, dyslipidemia, diabetes, smoking, sedentary lifestyle. Design: intervention study using random assignment for Web Group and App Group. The App Group was subsequently selected. Location: urban and rural health centres, teaching and non-teaching centres, in the Zaragoza I health area. Subjects: 261 people with BMI> 25 Kg/m2 and two extra cardiovascular risk factors. Interventions: 18 months follow up, initial checkups and after 1, 6, 12 and 18 months. The primary endpoints were: weight, blood pressure, tobacco consumption, cardiovascular events, quality of life and blood tests. Results: The control group completed the study in a higher proportion (79% vs 14%). The control and intervention groups attained a loss of 8% in and 5% median weight per year, respectively. After 6 months, the control group managed to reduce systolic and diastolic blood pressure by 7% and 5%, respectively. Glycosidic haemoglobin was 1% and 0.5% lower in the control and intervention groups, respectively (Wilcoxon Test=10; P= 0.089). Both groups reduced tobacco consumption (Wilcoxon=21; P=0.03). The control group had a higher prevalence of cardiovascular events. Quality of life improved (Wilcoxon Test=979; P=0.041). Conclusion: Patients visiting health centres to monitor weight obtain better results than those remotely assisted.(AU)

Introducción: Las enfermedades cardiovasculares constituyen la principal causa de morbimortalidad a escala mundial. Otra enfermedad con impacto significativo en los sistemas de salud pública es el VIH/sida. Los pacientes infectados con el virus de la inmunodeficiencia humana, tienen mayor riesgo de desarrollar enfermedades cardiovasculares. Posterior al tratamiento antirretroviral ha disminuido la incidencia de enfermedades oportunistas asociadas al VIH/sida, y se ha elevado la incidencia de enfermedades asociadas al envejecimiento, como la enfermedad cardiovascular. El uso de antirretrovirales inhibidores de la proteasa se asocia a hiperlipidemia y, por consiguiente, al aumento de complicaciones cardiovasculares. Objetivo: Determinar los marcadores lipídicos en pacientes con VIH/sida en tratamiento con inhibidores de la proteasa. Materiales y métodos: Se realizó un estudio observacional, analítico, transversal en los pacientes con VIH/sida de la provincia de Matanzas, durante el período comprendido entre marzo y agosto de 2020, en el Laboratorio Clínico del Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández. Resultados: Se evaluaron los marcadores lipídicos como factor de riesgo cardiovascular asociado al tratamiento antirretroviral, siendo los triglicéridos el de mayor incidencia. Conclusiones: Se identificaron los marcadores lipídicos como factores de riesgo cardiovascular, en este caso la hipertrigliceridemia.


Introduction: Cardiovascular diseases are the main risk of morbidity and mortality worldwide. Another disease with significant impact on public health systems is HIV/AIDS. Patients infected with the human immunodeficiency virus have a higher risk of developing cardiovascular diseases. After antiretroviral treatment, the incidence of opportunistic diseases associated with HIV/AIDS has decreased and the incidence of diseases associated with aging such as cardiovascular disease has increased. The use of protease inhibitor antiretroviral drugs is associated with hyperlipidemia and a consequent increase in cardiovascular complications. Objective: To determine lipid markers in patients with HIV/AIDS undergoing treatment with protease inhibitors. Materials and method: An observational, analytical, cross-sectional study was carried out in patients with HIV/AIDS from the province of Matanzas during the period from March to August 2020, in the Clinical Laboratory of the Provincial Teaching Clinical Surgical Hospital Faustino Pérez Hernandez. Results: Lipid markers were evaluated as cardiovascular risk factor associated with antiretroviral treatment, with triglycerides being the one of highest incidence. Conclusions: Lipid markers were identified as cardiovascular risk factors; in this case hypertriglyceridemia.

Introducción: Las enfermedades cardiovasculares ateroescleróticas están en el nivel máximo de las enfermedades que afectan tanto a mujeres como a hombres adultos mayores en todo el mundo. Objetivo: Evaluar las posibles diferencias de sexo y tipo de enfermedad aterosclerótica, en pacientes mayores de 60 años, fallecidos por enfermedad cardiovascular. Métodos: Se realizó un estudio descriptivo de corte transversal en 237 adultos mayores, fallecidos por enfermedad cardiovascular, perteneciente a tres áreas de salud del municipio Santiago de Cuba, en el período de enero a diciembre de 2021 Se analizaron variables socio-demográficas y clínicas. La información fue procesada y analizada mediante estadística descriptiva. Se sistematizó la información con el paquete estadístico versión 22 y Excel. Resultados: Existieron diferencias significativas en los factores de riesgos de tabaquismo y alcoholismo con una mayor frecuencia en la población masculina, la obesidad, el sedentarismo y la diabetes mellitus fue más relevante en las mujeres, con similar comportamiento en el infarto agudo de miocardio y la enfermedad cerebrovascular. Conclusiones: La mortalidad generada por las enfermedades cardiovasculares, en la población de adultos mayores, es similar para ambos sexos la presencia de factores de riesgo evidenciaron la misma frecuencia para la mortalidad por enfermedad del corazón y cerebrovascular(AU)


Introduction: Atherosclerotic cardiovascular diseases are at the highest level of diseases that affect both older women and men worldwide. Objective: To evaluate the possible differences of sex and type of atherosclerotic disease in patients over 60 years of age who died of cardiovascular disease. Methods: A descriptive cross-sectional study was carried out in 237 older adults who died from cardiovascular disease, belonging to three health areas of Santiago de Cuba municipality, from January to December 2021. Sociodemographic and clinical variables were analyzed. The information was processed and analyzed using descriptive statistics. The information was systematized with the statistical package version 22 and Excel. Results: There were significant differences in the risk factors of smoking and alcoholism with a higher frequency in the male population, obesity, sedentary lifestyle. Diabetes mellitus was more relevant in females, with similar behavior for acute myocardial infarction and cerebrovascular disease. Conclusions: The mortality generated by cardiovascular diseases, in the population of older adults, is similar in both sexes. The presence of risk factors showed the same frequency for mortality from heart disease and cerebrovascular disease(AU)

Introducción y objetivos: Las medidas de prevención cardiovascular hacen hincapié en la importancia del control de factores de riesgo cardiovascular (FRCV). Sin embargo, los estudios más recientes arrojan datos desalentadores cuyo impacto está por determinar. El objetivo de este estudio fue analizar la repercusión que los distintos FRCV y su grado de control tienen sobre el pronóstico de los pacientes tras un síndrome coronario agudo. Pacientes y métodos: Se recogieron datos epidemiológicos, farmacológicos y sobre el control de FRCV de 1.689 pacientes consecutivos que ingresaron durante 2018-2020 por síndrome coronario agudo en un hospital de tercer nivel. Finalmente, se calculó la tasa de eventos adversos cardiovasculares mayores. Resultados: Los pacientes que ingresaron por síndrome coronario agudo fueron predominantemente hombres, con índice de masa corporal>25kg/m2, fumadores (o exfumadores) y con un deficiente control de FRCV (50% para hipertensión y diabetes, y 35% en dislipidemia), especialmente aquellos pacientes con antecedentes personales de cardiopatía isquémica. Se encontró una infrautilización de fármacos útiles para dicho control. Se observó una relación directamente proporcional entre el número de FRCV (o su mal control) con la incidencia de eventos adversos cardiovasculares mayores a 2 años, siendo la hipertensión el factor con mayor repercusión cardiovascular. El confinamiento por SARS-CoV-2 empeoró el grado de control de FRCV y el pronóstico cardiovascular. Conclusión: Hay un importante margen de mejora en el control de los FRCV, que se traduciría en un beneficio pronóstico de los pacientes con cardiopatía isquémica. Es preciso intensificar las medidas de prevención y promoción de salud cardiovascular. (AU)


Introduction and objectives: Cardiovascular prevention measures place the emphasis on controlling cardiovascular risk factors (CVRF). However, the most recent studies provide disappointing data, the impact of which remains to be determined. The objective of this study was to analyse the impact that the different CVRFs, and their degree of control, have on the prognosis of patients after acute coronary syndrome. Patients and methods: Epidemiological, pharmacological, and CVRF control data were collected from 1,689 consecutive patients admitted from 2018 to 2020 for acute coronary syndrome to a tertiary hospital. Finally, the rate of major adverse cardiovascular events was calculated. Results: The patients admitted for acute coronary syndrome were predominantly men, with body mass index>25Kg/m2, smokers (or former smokers) and with poor CVRF control (50% for hypertension and diabetes and 35% for dyslipidaemia), especially those patients with a personal history of ischaemic heart disease. An underutilisation of useful drugs for CVRF control was found. A directly proportional relationship was observed between the number of CVRFs (or their poor control) and the incidence of major adverse cardiovascular events at 2 years, hypertension being the factor with the greatest cardiovascular impact. The SARS-CoV-2 lockdown worsened the degree of CVRF control and cardiovascular prognosis. Conclusion: There is still room for improvement in the control of CVRF, which would translate into a prognostic benefit for patients with ischaemic heart disease. The implementation of cardiovascular prevention campaigns seems essential. (AU)

Los informes de laboratorio tienen impacto en las decisiones médicas. El ayuno es un factor preanalítico "controlable" que influye en los distintos parámetros bioquímicos. El objetivo del presente trabajo es poner en discusión la realización en pediatría de análisis clínicos con la indicación de un ayuno fisiológico , analizando resultados obtenidos por diferentes autores y evaluando las diferencias clínicas encontradas según los criterios de calidad establecidos por el laboratorio de Química Clínica. La mayoría de los individuos durante el día se encuentran en estado postprandial. Los resultados del perfil lipídico en ayunas no representan las concentraciones reales promedios de los lípidos plasmáticos de un paciente. El ayuno no sería crítico en la etapa de pesquisa , pero puede ser relevante para establecer un diagnóstico certero o inicio de tratamiento. En el caso de la glucemia si se indica en el control rutinario del paciente, y no hay sospecha de alteraciones en el metabolismo de los hidratos de carbono la glucemia sin ayuno puede ser solicitada comparando la misma con valores de corte adecuado. Las diferentes guías nacionales e internacionales recomiendan que la elección de la métrica para la evaluación, control y seguimiento de pacientes con diagnóstico de diabetes se realicen según el objetivo terapéutico. En los trabajos analizados, observamos que varios parámetros bioquímicos presentaron diferencias estadísticas, aunque las diferencias clínicas no fueron relevantes y permanecieron dentro de los intervalos de referencia. El factor limitante para evaluar parámetros bioquímicos sin ayuno es la falta de valores de referencia adecuados. Hay evidencia suficiente para que tanto el perfil lipídico, la glucemia como el resto de los parámetros bioquímicos del laboratorio de química clínica, sean solicitados con la indicación de un ayuno fisiológico de 2, 4 o 6 horas, dependiendo siempre del motivo de consulta y/o la edad del paciente. Es esencial extender la evaluación a otros analitos en población pediátrica, así como evaluar nuevos puntos de corte para parámetros bioquímicos sin ayuno (AU)


Laboratory reports have an impact on medical decision-making. Fasting is a "controllable" preanalytical factor that influences the different biochemical parameters. The aim of this study is to discuss the performance of clinical analyses in pediatrics with the indication of physiological fasting, analyzing results obtained in different disciplines, and evaluating the clinical differences found according to the quality criteria established by the clinical chemistry laboratory. During the day, most patients are in a postprandial state. Fasting lipid profile results do not represent the actual average plasma lipid concentrations of a patient. Fasting would not be critical in the screening stage, but it may be relevant to establish an accurate diagnosis or initiate treatment. Regarding glycemia, if it is indicated in the routine control of the patient and there is no suspicion of alterations in carbohydrate metabolism, non-fasting glycemia can be requested, comparing it with adequate cut-off values. Different national and international guidelines recommend that the choice of metrics for the evaluation, control, and follow-up of patients with diabetes should be made according to the therapeutic objective. In the studies analyzed, we found that several biochemical parameters presented statistical differences, although the clinical differences were not relevant and remained within the reference range. The limiting factor in the evaluation of biochemical parameters without fasting is the lack of adequate reference values. There is sufficient evidence that the lipid profile, glycemia, and the remaining biochemical parameters of the clinical chemistry laboratory should be requested with the indication of a physiological fast of 2, 4, or 6 hours, always depending on the reason for consultation and/or the patient's age. It is essential to extend the evaluation to other analytes in the pediatric population, as well as to evaluate new cut-off points for biochemical parameters without fasting (AU)

Cardiovascular diseases (CVDs) remain the leading cause of worldwide death, accounting for significant morbidity, mortality, disability, and reduced quality of life. The global prevalence of cardiovascular (CV) risk factors, such as type 2 diabetes mellitus, hypertension, dyslipidemia, and obesity, has grown exponentially in the last decades, particularly in low-medium income countries, and it's projected to increase rapidly in the coming years as the population progressively ages, leading to increased cardiovascular disease (CVD) and associated mortality. In fact, data from the global burden of disease study shows that CV mortality, associated disability-adjusted life years (DALYs), and years of life lost (YLL) have increased steadily, nearly doubling from 1990 to 2019.Recent evidence proves the existence of an inverse association between hand grip strength (HGS), as a proxy for global muscle strength, with all-cause mortality, CV mortality, and the development of several chronic diseases. These associations have been demonstrated recurringly across the entire lifespan, beginning in childhood, and carrying on throughout adult life. Mounting evidence strongly indicates that HGS is an early predictor of chronic disease in premorbid populations and a therapeutic target for CVD prevention. Recent clinical trials have consistently shown that resistance exercise, which increases strength and potentially muscle mass, significantly improves the control of known CVD risk factors, reduces the risk of all-cause death and cardiovascular mortality.In this review, we explore the latest evidence regarding the association between low muscle strength and diverse metabolic alterations, along with the interventions that could improve cardiometabolic risk factors, while simultaneously increasing muscle fitness.(AU)


Las enfermedades cardiovasculares (ECV) siguen siendo la causa principal de muerte a nivel mundial, con una gran significación en términos de morbilidad, mortalidad, incapacidad y reducción de la calidad de vida. La prevalencia global de factores de riesgo cardiovascular (CV), tales como diabetes mellitus tipo 2, hipertensión, dislipidemia y obesidad ha crecido exponencialmente en las últimas décadas, particularmente en los países con rentas bajas-medias, previéndose que se incremente rápidamente en los años venideros a medida que la población envejezca, originando un incremento de las ECV y la mortalidad asociada. De hecho, los datos procedentes del estudio sobre carga global de la enfermedad muestran que la mortalidad CV, los años de vida ajustados por discapacidad (DALY), y los años de vida perdidos (YLL) asociados se han incrementado de manera constante, y casi doblándose desde 1990 a 2019.La evidencia reciente prueba la existencia de una asociación inversa entre la fuerza de agarre (HGS), como indicador de la fuerza muscular global, y la mortalidad por todas las causas, la mortalidad CV y el desarrollo de diversas enfermedades crónicas. Estas asociaciones han sido demostradas recurrentemente durante todo el periodo de vida, comenzando en la escuela, y prosiguiendo durante toda la vida adulta. La evidencia creciente indica enfáticamente que la HGS es un factor predictivo temprano de enfermedades crónicas en poblaciones premórbidas, y un objetivo terapéutico para la prevención de las ECV. We hypothesized that a poorer cardiovascular health status is related to a higher risk of hypertension-mediated organ-damage (HMOD) or hypertension-related comorbidities (HRC). We assessed the relationship between cardiovascular health metrics (CVHM) and HMOD-HRC in 243 hypertensive patients from primary care center followed for two years. We recorded the baseline CVHM score (Life's Simple 7) plus clinical data, including prevalent and incident HMOD-HRC, hospitalization and mortality. The prevalence of ideal CVHM scores was very low in both men and women. The patients with healthier CVHM scores were younger, and had a lower prevalence of diabetes, cardiovascular disease and chronic kidney disease. We recorded 264 cases of HMOD-HRC (225 at baseline and 39 during follow-up). Nine patients died and 64 had any-cause hospitalization during follow-up. A lower prevalence of HMOD-HRC and unfavorable outcomes was observed as the number of ideal CVHM increased (P<0.05). Multivariate logistic regression adjusted for confounders showed a lower CVHM score (0-1) was associated with increased odds of HMOD-HRC (4.04, 95% CI 1.26-12.94; P=0.019) and composite endpoint (HMOD-HRC, death or all-cause hospitalization) (3.43, 95% CI 1.19-9.92; P=0.023). Individual components were less predictive than the cumulative CVHM score. Few hypertensive patients in this urban population had ideal CVHM scores. An inverse relationship between scores and outcomes (HMOD-HRC, death or hospitalizations) was observed. Interventions to increase this score may improve prognosis among community-based hypertensive patients.

ABSTRACT

El objetivo de este estudio fue establecer relaciones entre las variables de rendimiento físico e índices antropométricos según la edad de bomberos adultos chilenos, en servicio activo, provenientes de la región de Valparaíso. Participaron 68 bomberos, hombres, sanos, en servicio activo, que fueron divididos en dos grupos según edad; 30 años (n = 32; 40,6 ± 8,5 años). Se evaluaron las variables antropométricas de masa corporal (MC), estatura, perímetro de cintura (PC), índice de masa corporal (IMC] = Peso /estatura2 [m]) e índice cintura-estatura (ICE). La capacidad de salto vertical fue evaluada con los protocolos de Squat Jump, Countermovement Jump y Abalakov Jump. La resistencia en carrera fue estimada con la prueba de Course Navette y se calculó indirectamente el consumo de oxígeno (VO2). Las variables antropométricas para el grupo > 30 años fueron mayores en comparación al grupo de menor edad en MC (p = 0.027), IMC (p = 0.015), PC (p 0.05) entre ambos grupos. Existió una correlación significativa positiva entre la edad y las variables de MC (r = 0,252), IMC (r = 0,307), ICE (r = 0,431) y PC (r= 0,401). Al comparar ambos grupos de edad hubo diferencias antropométricas, pero no en la condición física. Se sugiere reforzar programas de entrenamiento para optimizar la composición corporal y capacidad física de bomberos en servicio activo para responder a las exigentes tareas que demanda este ámbito laboral.


SUMMARY: The aim of this study was 1) to compare anthropometric characteristics, jumping ability, and running endurance according to age and 2) to determine the relationship between age and physical performance parameters in Chilean adult firefighters in active service from the Valparaíso region. Sixty-eight healthy male firefighters, in active service, were divided into two groups according to age; 30 years (n = 32; 40.6 ± 8.5 years) participated. The anthropometric variables of body mass (BM), height, waist circumference (WC), body mass index (BMI = weight/height2 [m]) and waist-to-height ratio (WHR) were evaluated. Vertical jumping ability was assessed with the Squat Jump, Countermovement Jump and Abalakov Jump protocols. Running endurance was estimated with the Course Navette test and oxygen consumption (VO2) was calculated indirectly. Anthropometric variables for the > 30 years group were higher compared to the younger age group in BM (p = 0.027), BMI (p = 0.015), WC (p 0.05). There was a significant positive correlation between age and the variables of BM (r = 0.252), BMI (r = 0.307), WHR (r = 0.431) y WC (r= 0.401). When comparing both age groups, there were anthropometric differences, but not in physical condition. It is suggested to reinforce training programs to optimize the body composition and physical capacity of firefighters in active service in order to respond to the demanding tasks demanded by this work environment.

ABSTRACT

Objective: To analyze the agreement between body mass index (BMI) and waist-to-height Ratio (WHtR) to identify preschool and school children with cardiovascular risk factors (CRFs). Methods: Three-hundred-twenty-one kids were divided into preschool (3-5 years) and school children (6-10 years). BMI was used to classify children as overweight or obese. Abdominal obesity was defined with a WHtR ≥0.50. Fasting blood lipids, glucose and insulin were measured, and the homeostasis model assessment of insulin resistance (HOMA-IR) was calculated. The presence of CRFs and multiple non-waist circumference (non-WC) metabolic syndrome factors (MetS-Factors) [high HOMA-IR, high triglycerides and low high-density lipoprotein cholesterol (HDL-C)] were analyzed. Results: One-hundred-twelve preschool and 209 school children were evaluated. WHtR ≥0.50 classified abdominal obesity in more than half of the preschool children, exceeding those classified with overweight+obesity by BMI (59.5% vs. 9.8%; p0.05). There were similar proportions of school children classified with abdominal obesity by the WHtR and overweight+obesity by the BMI (18.7% vs. 24.9%; p>0.05). There was substantial agreement between WHtR and BMI to identify school children with high total cholesterol values, low-density lipoprotein cholesterol (LDL-C), triglycerides, non-HDL-C, insulin, HOMA-IR, low HDL-C values, and the presence of multiple non-WC MetS-Factors (kappa: 0.616 to 0.857, p<0.001). Conclusion: In preschool children WHtR ≥0.5 disagree with BMI results, but in school kids, it has good agreement with the BMI to classify the children´s nutritional status and to identify those with CRFs.


Objetivo: Analizar el grado de acuerdo entre el índice de masa corporal (IMC) y la relación/cintura estatura (rCE) para identificar niños con factores de riesgo cardiovascular (FRC). Métodos: Estudio transversal analítico con 112 niños preescolares (3-5 años) y 209 escolares (6-10 años). El sobrepeso y la obesidad se clasificaron con el IMC y la obesidad abdominal con la rCE ≥0.50. Se tomaron muestras sanguíneas en ayuno para análisis de glucosa, insulina, lípidos y cálculo del índice HOMA-IR. Se analizó la presencia de FRC y de múltiples factores del síndrome metabólico (Factores-SinMet) diferentes a la cintura [HOMA-IR elevado, triglicéridos elevados y colesterol de alta densidad (HDL-C) bajo]. Resultados: rCE ≥0.50 clasificó con obesidad abdominal a más de la mitad de los niños preescolares, excediendo el número de niños clasificados con sobrepeso+obesidad por IMC (59.5% vs 9.8%; p0.05). Fueron similares las proporciones de niños escolares clasificados con obesidad abdominal por la rCE o con sobrepeso+obesidad por el IMC (18.7% vs. 24.9%; p>0.05). Hubo acuerdo sustancial entre la rCE y el IMC para identificar niños escolares con valores elevados de colesterol total, colesterol de baja densidad, triglicéridos, colesterol no-HDL, insulina, HOMA-IR, valores bajos de HDL-C y la presencia de múltiples Factores-SinMet diferentes a la cintura (kappa: 0.616 a 0.857, p<0.001). Conclusión: En niños preescolares la aplicación de rCE ≥0.5 no presenta acuerdo con el IMC, pero en escolares presenta un acuerdo sustancial con el IMC en la clasificación del estado nutricional y en la identificación de niños con FRC.

Introducción: En la vejez, el valor predictivo de los factores de riesgo tradicionales es inconsistente. Con el aumento de la edad, las asociaciones entre los factores de riesgo tradicionales y las enfermedades cardiovasculares cambian. Objetivo: Recopilar información sobre los factores de riesgo tradicionales de enfermedad cardiovascular y su valor predictivo en el adulto mayor. Métodos: En el policlínico "Ramón López Peña" de Santiago de Cuba, en el período comprendido entre enero y mayo de 2022, se realizó una revisión sobre el tema. La búsqueda se realizó a partir de las bases de datos Pubmed, Infomed y SciELO, sin restricción de fechas, en los idiomas español e inglés. Resultados: Los hallazgos más recientes sugieren que los factores de riesgo tradicionales para la enfermedad cardiovascular podrían ser predictores más débiles del riesgo futuro en los ancianos, mientras que la fuerza de otros factores aumenta con la edad. Las personas mayores constituyen un subgrupo especial en el que otros factores como la polifarmacia, la hipotensión ortostática, la depresión y la condición física han demostrado ser determinantes potencialmente importantes de su riesgo cardiovascular. Conclusiones: El valor predictivo de la presión arterial, el colesterol y la obesidad en los ancianos disminuye o incluso se invierte, mientras que otros como la polifarmacia, la fragilidad y el deterioro cognitivo parecen ser prometedores.


Introduction: In old age, the predictive value of common risk factors is inconsistent. With increasing age, the associations between common risk factors and cardiovascular disease change. Objective: To collect information on the common risk factors for cardiovascular disease and their predictive value in the elderly. Methods: A review on this subject was carried out at Ramón López Peña community clinic in Santiago de Cuba, from January to May 2022. The search was carried out from Pubmed, Infomed and SciELO databases, with no date restrictions, in Spanish and English. Results: The most recent findings suggest that common risk factors for cardiovascular disease might be weaker predictors of future risk in the elderly, while the strength of other factors increases with age. Older people constitute a special subgroup in which other factors such as polypharmacy, orthostatic hypotension, depression, and physical condition have been shown to be potentially important determinants of their cardiovascular risk. Conclusions: The predictive value of blood pressure, cholesterol and obesity in the elderly decreases or even reverses, while others such as polypharmacy, frailty and cognitive impairment appear to be promising.

ABSTRACT

INTRODUCTION AND OBJECTIVES: Cardiovascular prevention measures place the emphasis on controlling cardiovascular risk factors (CVRF). However, the most recent studies provide disappointing data, the impact of which remains to be determined. The objective of this study was to analyse the impact that the different CVRFs, and their degree of control, have on the prognosis of patients after acute coronary syndrome. PATIENTS AND METHODS: Epidemiological, pharmacological, and CVRF control data were collected from 1,689 consecutive patients admitted from 2018 to 2020 for acute coronary syndrome to a tertiary hospital. Finally, the rate of major adverse cardiovascular events was calculated. RESULTS: The patients admitted for acute coronary syndrome were predominantly men, with body mass index>25Kg/m2, smokers (or former smokers) and with poor CVRF control (50% for hypertension and diabetes and 35% for dyslipidaemia), especially those patients with a personal history of ischaemic heart disease. An underutilisation of useful drugs for CVRF control was found. A directly proportional relationship was observed between the number of CVRFs (or their poor control) and the incidence of major adverse cardiovascular events at 2 years, hypertension being the factor with the greatest cardiovascular impact. The SARS-CoV-2 lockdown worsened the degree of CVRF control and cardiovascular prognosis. CONCLUSION: There is still room for improvement in the control of CVRF, which would translate into a prognostic benefit for patients with ischaemic heart disease. The implementation of cardiovascular prevention campaigns seems essential.
